Everyone has the right to housing and healthcare.

Support House is a community-based non-profit with more than 42 years of experience delivering supportive housing, healthcare access, peer support, and outreach across Halton Region. We support people facing complex mental health, substance use, housing, and health concerns through person-directed, inclusive, and trauma-informed services.

We deliver a connected continuum of supports that help people stabilize, access care, and move forward. This includes 24/7 staffed supportive housing, transitional and semi-independent housing, rent supplements, housing stabilization supports, and community outreach. All housing referrals flow through Halton Region’s coordinated access system, ensuring equitable access and continuity as needs change.

Support House is also expanding access to primary care through our Mobile Health Team and Community Health Centre model. Our interdisciplinary team brings primary care, nursing, social work, and peer support directly into the community, supporting people who are unhoused, shelter-based, or face barriers to traditional healthcare.

Through the Centre for Innovation in Peer Support, we strengthen individual and system capacity by delivering peer services, training, mentorship, and co-designed approaches grounded in lived and living experience.

Guided by equity, collaboration, and community partnership, Support House works alongside health systems, municipalities, and local organizations to build sustainable solutions that improve access to housing, healthcare, and belonging.
